Case Management

Case Management is a consumer-driven service intended to empower and assist individuals and their families in making choices concerning in-home, community-based or long-term care services. Services are provided by licensed social workers to individuals and their caregivers, in order to assess the needs of the person and to arrange, coordinated and monitor services to meet those needs. Case managers are a centralized evaluator of client needs and arranger of services. With the client, they develop and implement a service plan to help fill the unmet needs of the individual through both formal and informal resources and services. Case Managers coordinate service delivery between multiple agencies, individuals and others. They act as an agent for the client and not as a promoter for entities providing such services.

Home Delivered Meals (Meals on Wheels)

Meals delivered to elderly homebound. Meals must meet one-third of the recommended dietary allowance. The program relies on a suggested donation from participants to meet the overall operational requirements.

Homemaker Services

Homemakers provide assistance to eligible individuals in their own homes to restore, enhance, or maintain their capabilities for self-care and independent living. Assistance is provided for light housekeeping, shopping, and laundry. Homemakers may also assist those individuals who do not have nursing needs with bathing assistance. Cost is based on a sliding fee scale.

Short-Term Respite

Respite volunteers provide companionship for homebound clients so the caregiver can attend to personal business or recreational interests outside the home. This allows caregivers intervals of needed relief.

Family Caregiver Respite

Provides respite care to enable unpaid caregivers temporary relief (minimum of 4 hours) from their caregiving responsibilities. Respite may be in the form of in-home, adult day care, or facility respite for an overnight stay on an intermittent or occasional basis.

Nutrition Advisors

An in-home program for nutritionally at risk elderly, updating nutrition, food safety, menu planning and money management knowledge
